What do I need to complete an application

To complete the application the following must accompany this form:

• Police check, less than three (3) months old.
• complete all of the relevant sections of the application form
• Two character references
• Scale of fees
• Notice of employment offered 
• Certificate of Registration of a Business Name
• Resume 
• Licence fee
