On (date) I (State what the issue is, for example, “a dispute with management” or “entered into a contract with your company” or required service, for example, “physiotherapy was not provided for as per my contract”).
Unfortunately, (detail what the specific problem is, for example: “the dispute resolution outcome has been disappointing” or “the contract is not correct”. Specify the service you are talking about, for example, “the physiotherapy services guaranteed”).
To resolve this issue, I would appreciate it if you could (say what you want, for example, “review the dispute procedures” or “review the contract” or “provide the service requested”).
Enclosed are copies of relevant records (include copies of any relevant records, for example copy of the contract, letters and any other documents).
