Associations fees, forms and online transactions
Please use the AssociationsOnline portal for all forms and transactions. If you have difficulties, you can use the downloadable forms listed below.
You can manage your association online including updating details and completing forms.
The application can be made through the AssociationsOnline portal. Alternatively, the completed forms can be mailed to Locked Bag 100, EAST PERTH WA 6892. For customer enquiries or questions about our forms, Associations staff can be contacted by telephoning the Associations Information Line on 1300 304 064 (or 08 6552 9300).
Fees and forms
The fees listed below were correct as at 1 July 2019. Fees are subject to annual review and may change without notice.
2021 fees update
Please note there will be an increase in the prescribed schedule fees for Associations and Clubs applications which will take effect on 01 July 2021.
The new fees schedule will be available to view in the table below from 01 July 2021.
