Co-operative form 03 - Application to register a co-operative

This form should be lodged within two months after the formation meeting. If needed, you can apply for a longer timeframe using the Form 12 - Application for an extension or abridgement (shortening) of time

Before applying to register a co-operative its rules and, if necessary, its formation disclosure statement must be approved by the Registrar of Co-operatives. If you have not already done this, before submitting this form lodge the Form 1 - Application for approval of proposed rules and formation disclosure statement.

What do I need to complete the form

To complete the submission of the form you must:

  • Complete all the relevant sections of the form.
  • Make sure you have provided all supporting documents.
  • Sign the declaration.
  • Pay the application fee.

The following supporting documents must also accompany this form:

  • Two copies of the proposed rules, as approved by the Registrar, signed and certified by the chairperson and secretary of the formation meeting as having been agreed to by members at the meeting.
  • A copy of the formation disclosure statement, as approved by the Registrar, signed and certified by the chairperson and secretary of the formation meeting to have been presented to members (necessary if the co-operative is distributing).

Once we receive your form you will be issued with a Payment Number (PN) so you can make payment using our secure online payment portal

Pay by cheque or money order:

Attach a cheque or money order made out to "Department of Energy, Mines, Industry Regulation and Safety" to the completed form.
