This form must be used for applying to the Building Commissioner (or his/her authorised delegate) for registration as an adjudicator or review adjudicator (or both) under the Building and Construction Industry (Security of Payment) Act 2021.

Registration as an adjudicator or review adjudicator entitles an individual to adjudicate adjudication applications and adjudication review applications respectively.
