This form is used to advise the vehicle or motorcycle is not covered by a statutory warranty and describes certain particulars about the vehicle. This form is to be displayed in the windscreen of a vehicle, or attached to a motorcycle.

A copy of the form, signed by the purchaser, must be retained for two years after the sale of a vehicle.
